Terms and Conditions of Service

This website is operated by Salveo Forma Physiotherapy. Please read the terms and conditions carefully. By using the website, browsing the site in any manner or using our services, you agree to be bound by these terms and conditions. The terms “we”, “us” and “our” refer to our business.

 

Service Booking

Following your booking request, we will confirm details of your appointment, including the date, time and assigned physiotherapist.

 

Cancellation Policy

We understand you may need to cancel your appointment at short notice due to illnesss and unexpected events and we try our best to be flexible around your requirements and needs. However, in order to maintain service efficiency:

- There is no charge for appointments cancelled with more than 48 hours notice.

- For cancellations within 48-24 hours you will be charged 50% of the appointment fee.

- For cancellations within 24 hours you are required to pay the full appointment fee.

- If our therapist has arrived at your location and travel expenses have previously been agreed then the full travel expenses will be added to the cancellation fee.

Travel fees

A complimentary travel allowance covers journeys up to 5 miles from our therapist's home location. Beyond this radius, additional mileage is calculated at £2.00 per mile one way. This charge reflects the actual distance travelled. For example if you live 10 miles away you may be charged £10. This will be pre-arranged before your visit appointment. If the therapist is visiting other clients in the same area the travel charge may not apply.

 

Documentation

Standard note writing/admin time is included in the price of your session and will be completed after the appointment has finished.

If any complex administrative work such as detailed therapeutic assessments, care plans, case reports or equivalent is required this will be at an additional cost and agreed prior to completing.

 

How to pay

We offer multiple payment options including bank transfers and cheques. Invoices are generated monthly and can be provided electronically via email or in hard copy according to your preference.

Invoices must be paid within 30 days of receipt.

 

Service discontinuation

We reserve the right to refuse service to anyone at any time for any reason.

 

Pricing

Service rates are subject to periodic review. Clients will receive advanced notice of any changes. The initial assessment has a set fee and follow up appointments are charged based on the time the therapist has spent with the client.

 

Availability of services

Service delivery may be affected by scheduled leave, professional development, or holidays. Alternative arrangements will be proposed for affected appointments as able.
